The Official Cocurricular Record can be issued only with the student's written consent. To request a transcript, open the Transcript Request Form and type in all requested information. Print and sign the form, then mail or fax it to the Dean of Student Affairs Office at the following address:
Dean of Student Affairs Office
Truman State University
100 E. Normal Street
Kirksville, MO 63501
Fax: (660)785-5403
Transcripts may also be requested in person at the Student Affairs Office, 3100 Student Union Building. Requests for an Official Cocurricular Record cannot be made by e-mail or by phone.
Release:
The Official Cocurricular Record may be released directly to the student or to a third party. Records that are issued to a student will be marked "Issued to Student" and will remain official as long as they are not opened. Any request to send a record to a third party must include a complete address. If an incorrect address is provided, the student will be responsible for ordering a new Record.
Cocurricular Record Fees:
Official Cocurricular Records cost $5.00 each. Records can also be faxed for an additional $3.00 charge, or sent overnight mail for an additional $10.00 charge. Additional fees will be charged for international overnight service; please contact the Dean of Student Affairs Office for an estimate if you would like to pursue this option.
Payment must be made before records will be released. Payment can be made by sending a check payable to Truman State University, or by using your Visa, Master Card or Discover Card. To charge Cocurricular Record fees, include your credit card number and expiration date with your request.
Processing Time:
All transcript requests are processed in the order in which they are received. Please note that a request to send a transcript by fax or overnight mail does not expedite the processing of your request, although it will significantly reduce delivery time. Normal processing time is one to two working days.
Questions: If you have questions about ordering records or the status of your transcript request, please contact the Secretary to the Dean of Student Affairs at (660) 785-5404. The office is open Monday through Friday 8:00 a.m. to 5:00 p.m.
